December Thrive: County Home Visiting Programs and Help Me Grow

When: Thursday, December 11, 2025 | Lunch at Noon, Program 12:30 – 2:00 PM

Where: Holiday Inn, 200 W. First Street, Duluth, MN 55802 – lower level ballrooms

Who: Thrive participants and others who work with families with young children such as mental health providers, family support (parenting education, outreach workers, foster care support, etc.), healthcare providers (pediatricians, nurses, doulas) and early care and education professionals

Event Description: We’ll start with a soup-and-salad lunch followed by the program.

First, Spoorthy Uddurhally from the Carlton-Cook-Lake-St. Louis County Community Health Board will share information about the home visiting programs offered through the community health board across three counties, free of cost to families in Northern and Southern St. Louis County, Carlton, and Cook County.

The board offers several Home Visiting Programs to meet the needs of families, including the universal home visiting program, the evidence-based MECSH model, and the Heart-to-Heart program, which is specifically designed for pregnant mothers with hypertension.

All the home-visiting models start prenatally and are delivered in partnership with registered nurses. The funding for the program comes from the Strong Foundations grant, administered by MDH and MCH/TANF. Through these programs, families are empowered to navigate the transition to parenthood with confidence and access to resources. It offers support starting during pregnancy and continues through the child’s second birthday.

In addition, Kacy Jacobs from Help Me Grow will share about their work during this session.
